1. Yup! The infirmary is always willing to give to those in need, and there are those who enjoy being fed upon for the more "live" sorts... There's a PC-run establishment for Vampires called Desmodus Mori that specializes in serving them as well. One can look into an apothecary's hut for a more mobile, bottled source of creature blood if they'd like too.

2. The first. In order of effectiveness, it goes Witch -> Non-practicing Human -> Magical Creature (found in the Wilde, for example, but some are sold as pets) -> Monster -> Regular Animal. A vampire would need a good deal more blood to be wholly satisfied on the right side of the scale, while they may not need a quarter as much with the left side; Magical Creatures are the middle ground, essentially.

3. It's very fair to assume there would be no intentional death! The body feeds, instinctively, until it's satisfied or until they feel their "source" draining; it's better to have an alive source of food than a dead one, so while the victim will be in poor shape they will get better in time, unless one has not fed for an extremely long time... But please do communicate to us if that is the case.

4. This would be to player's discretion, as the size of their feral form as well as what traits enhance each time is up to you. If you'd like them to be as small as a normal bat or the size of Batman himself, then either is fine and valid!
